Find forms

Use the tabs, quick filters, or search to find the forms and information you need.

  1. Go to the Forms page.
  2. Use the tabs, quick filters, or search to find forms.
    • Tabs: Forms are organized under tabs according to their form type. Select a tab to view all forms of the same form type.
    • Filter by error code: Select to review all forms with errors or select a specific error code from the dropdown.
    • Filter by last modified by: Select a user from the dropdown.
    • Filter by payer ID: Select a payer ID from the dropdown.
    • Originals: Forms that have not needed to be corrected.
    • Corrections: Forms that were previously transmitted and then had to be corrected.
    • Search: Enter a name, TIN, or account number in the Search bar. You must enter at least three letters or numbers.

Customize the Forms page

Find out how to select and arrange the fields shown on the Forms page.

  1. On the Forms page, click the Columns icon.
    The Columns icon is highlighted.
  2. Click the Crossed-out eye icon next to a column to hide it from view. Click the regular Eye icon to show a column.

  3. Drag the columns to rearrange their order of appearance in the table.
  4. Click Apply. The table reloads to display your preferences.
    Tip: To sort the form data by ascending or descending values in a specific column, click the column header.
