1099 Reporting

The ACA form filing workflow

Follow these steps if you are filing ACA forms to the IRS and states.

ACA forms are IRS information returns used to report health coverage provided to employees. There are two types: 1095 forms (returns) and 1094 forms (transmittals). The IRS uses these forms to verify compliance with ACA employer requirements and to determine employees' eligibility for premium tax credits.
1095 forms
These forms document who was offered and enrolled in health coverage by employers:
  • 1095-B forms are filed by any entity that provides minimum essential health coverage, such as insurers or small self-insured employers. They report coverage provided to individuals during the calendar year.

  • 1095-C forms are filed by larger employers with 50 or more full-time employees. They report information about each employee, specifically offers of health coverage to full-time employees and their families, and enrollment in coverage for employers that sponsor self-insured health plans.

1094 forms
These are transmittals that deliver the 1095 forms to the IRS:
  • 1094-B forms transmit the 1095-B returns to the IRS.

  • 1094-C forms report summary information for each employer and transmit the 1095-C returns to the IRS.
