Add a single form

Add a form to make it available for printing and transmitting.

Each form is linked to a payer and a recipient, so payer and recipient information must be in the system before you can add forms. Import or manually add payer and recipient data first, then add their forms.

  1. On the Forms page, click Add a form and select a form type from the dropdown.
  2. Add payer and recipient information.
    1. To add payer information, click Add and search by name or TIN.
    2. To add recipient information, click Add and search by name, TIN, or account number.
      Tip: To search by TIN, enter nine digits with no dashes, for example, 123456789. To search by name or account number, enter at least three characters.

  3. Click into the fields to enter values.
    If you are adding a W-2 form:
    • In Box 12, you can enter five different amounts.

    • In Box 14, you can enter four different amounts.

    • In Boxes 15 and 20, you can enter information for one state and one locality.

  4. Click Save.
